Job Description

Through directing and managing a multi-disciplinary team of home health care clinicians and support staff, Clinical Managers focus on care coordination, financial management, and resource utilization to yield cost-effective outcomes that are patient-centric, safe, and provided in the least restrictive setting. Clinical Managers provide the environment where patients, families, and stakeholders are viewed as essential members of the team, costs are contained, and enhanced outcomes are achieved. Clinical Managers provide a fluid and dynamic practice that is most effective when it changes and adapts with the challenges of the healthcare system. Stewardship of the healthcare dollars, safe transitions of care, evaluating patient adherence, and consistent stakeholder communication are critical interventions that Clinical Managers employ, while maintaining a primary and consistent focus on quality of care and patient self-determination. Have an understanding of utilization management and payer requirements in home health. Salary Range: $125,000 - $140,000 Annual Status: Full-time Benefits: Medical, Dental, Vision, Life Insurance, 403(b) Education/Experience/Qualifications: Possession of a Registered Nurse License (RN) to practice healthcare in the State of California and current CPR certification, with at least 2 years of recent experience in home care, preferably with 1 year of supervisory experience or equivalent training in a home health or hospice agency. Excellent communication, leadership, organizational, decision-making, and time-management skills. Working knowledge of: Title XXII, HIM-11, Medi-Cal requirements, agency policies and procedures.
